Elbow Arthritis

Elbow arthritis is less common than hip or knee arthritis, but can be significantly disabling, particularly in manual workers, athletes, and those with a history of elbow injury. It causes progressive pain, stiffness, and loss of movement that affects daily activities and quality of life.

Types of Elbow Arthritis

Primary osteoarthritis

Gradual wear of joint cartilage with ageing and use. More common in men performing heavy manual work. Tends to affect end ranges first, with the mid-arc remaining comfortable for many years.

Post-traumatic arthritis

Develops after a previous elbow fracture, dislocation, or ligament injury, even years later. The most common cause of elbow arthritis in younger patients.

Rheumatoid arthritis

Autoimmune destruction of the joint lining (synovium) leading to cartilage and bone erosion. Affects both elbows in most patients. Typically presents at a younger age.

Osteochondritis dissecans

Affects young throwing athletes and gymnasts, a cartilage and bone fragment separates from the capitellum due to repetitive compression. Can progress to arthritis if untreated.

When articular cartilage is destroyed, the underlying bone is exposed, creating bone-on-bone contact. The body responds by forming osteophytes (bone spurs) at the olecranon, coronoid, and radial head. These spurs mechanically block movement and can break off to form loose bodies that cause the elbow to lock or catch. The elbow joint is particularly unforgiving of osteophytes due to its tight bony congruity.

What changes in the Arthritic Elbow?

Stages of Progression

Early

Cartilage thinning, small osteophytes beginning to form. Loss of last 20-30° of extension and flexion. Pain at end-range only, mid-arc comfortable.

Moderate

Large osteophytes, significant joint space narrowing, loose bodies common. Range limited to 50-100°. Pain beginning with everyday activities, not just end-range.

Severe

Bone-on-bone contact, grossly deformed joint surfaces. Range often <50°. Constant pain at rest and activity. Possible ulnar nerve palsy from osteophyte compression.

Symptoms

  • End-range stiffness - loss of full extension (straightening) and flexion (bending)

  • Activity pain - at end of movement and during heavy use; mid-arc often pain-free early on

  • Locking and catching - loose bodies can cause the elbow to suddenly lock mid-movement

  • Tingling in ring and little fingers - osteophytes can compress the adjacent ulnar nerve

⚠ Pain and stiffness at the elbow can be due to elbow arthritis but also can be mimicked by added tingling and numbness due to ulnar nerve compression caused by bony spur formation. Seek the advise of your surgeon if concerned.

Pre-Operative Preparation

Activity modification and analgesia

Avoid heavy lifting and repetitive loading. Regular paracetamol and NSAIDs manage pain. Topical anti-inflammatories are useful for flares with minimal systemic side effects.

Physiotherapy

Tailored exercise maintains range of movement and builds periarticular muscle strength. Heat before exercise and ice after activity help manage pain. Physiotherapy does not reverse arthritis but significantly improves function.

Intra-articular injection

Corticosteroid injected directly into the elbow joint provides 3-6 months of pain relief and reduced swelling, particularly effective in rheumatoid arthritis flares. Hyaluronic acid is an alternative with potentially longer duration.

Arthroscopic debridement and loose body removal

Keyhole surgery to remove loose bodies, trim osteophytes, and wash out the joint. Highly effective for early-to-moderate arthritis, relieves locking, improves range of movement. Results typically last 3-5 years before further intervention is needed.

Total elbow replacement

For severe end-stage arthritis, primarily in rheumatoid disease or older, lower-demand patients. Effective for pain relief but strict activity restrictions apply for life (no lifting >2-5 kg). Higher complication and revision rate than hip, knee or shoulder replacement.

⚠ Total elbow replacement is generally reserved for patients over 65, or for rheumatoid arthritis with severe joint destruction. In younger patients, joint-preserving surgery is strongly preferred. Strict lifetime weight restrictions mean it is not appropriate for anyone wishing to return to manual work or sport.

Recovery After Arthroscopic Debridement

Elbow arthroscopy is technically demanding, many important nerves and vessels lie close to the joint. It is important to choose a surgeon with specific elbow arthroscopy experience, as the risk of nerve injury is higher than in shoulder or knee arthroscopy.
